Chapman Hills Elementary's 2017-18 Registration Checklist


Please make sure your child returns the following items by Friday, September 2, 2016.  It is necessary to have one emergency card for each child.  Remember to update these cards throughout the school year when changes occur.  We must be able to contact someone on the card in case of an emergency.  If a person is not on the card, he/she will not be allowed to sign out your child.  Current telephone numbers are very important

_____  Over the Counter Product Authorization- Print and sign form.

_____  Medication Authorization Form- Print and sign form if applicable.

_____  Parent-Student Handbook (review with students)- Review handbook with your child.  

_____  Insurance Verification form- Print and sign cover letter, brochures will be sent home on the first day of school.

_____  Native American Education Program (optional)- Print and sign form if applicable.

_____  Earthquake Kit

_____  Notification of OUSD Promotion Policy
